What: After cleaning the vacuum truck the hatch was not fixed properly. When truck came to DB for loading the fluid, I noticed that top hatch on vacuum truck was not closed properly, operators were going to connect hoses and start transferring process. 

Action: Stopped the job and informed operators that need to close and fix the hatch on top side of vacuum truck then start the process in safe manner.
